I'm thinking something to with hidden/visible attributes...

Why is this happening and how do I disable it?

This occurs with any new profile created.

Additionally there is a desktop.ini in the Programs folder from the start
menu.

I've searched around but found no definite resolve.

Not a resolution, but a temp fix for the RIS'd machine: Create a default
user that has all the desktop.ini files (find them by a search) hidden again,
so that any new users will have them hidden automatically on creation of
their new account. I don't know if this resetting of the hidden attribute
will stick if this machine is imaged for other computers.
