Numbering with hyperlink Table of Content

I have a document with a Table of Contents which is hyperlinked. I have
updated the document and Table of Contents to reflect additional numbering.
I cannot seem to fix the following (example below):
a) 1.0 should be before 'GENERAL NAVIGATION', 4 is the page number.
b) 'User Interface' should be in between 1.1 & page number 4.
I would appreciiate any help I can get.

Inspect the two relevant headings in the document (not the TOC entries) to
see if you have an empty paragraph before each one that has the
corresponding heading style applied. If so, delete it. Or, if there is a
manual page break before either of these headings, delete it and either
format the heading as "Page break before" (Format | Paragraph | Line and
Page Breaks) or format it and enough following paragraphs as "Keep with
next" to push it to a new page.

I honestly don't know what I did, but by trying to figure out your message
below I worked out my numbering.
